I affirm that I have read and am in agreement with the Love One Statement of Faith.

I understand that I may from time to time become aware of confidential information relating to those served by Love One. I agree to guard the privacy of those we serve and will not disclose any such information outside of the ministry.

I understand that, depending on the type of volunteer work I am involved in, I may be required to complete a background check and am willing to sign a separate waiver to that effect.
